When young people turn 18, there are changes in terms of the services that are available to them, the law and the level of benefits available. The period in which a young person prepares for and experiences these changes is known as ‘transition’, as they move from childhood to adulthood. This can be a difficult time for young people and their parents/carers, so it is important that the right support is in place to reduce anxiety and ensure a smooth transition.

However, it has been identified that there is a need to improve the support offered for young people with additional needs during transition.  This has been identified by staff within the local authority and the NHS, as well by as Ofsted and CQC.

Therefore, staff from Leicester City Council and the NHS have been working in partnership to develop a ‘Joint Health, Social Care and Education Transitions Strategy’ which sets the direction of travel and outlines the priorities and actions that are needed for improvement.  The strategy is supported by an  delivery plan which further details the exact work which will need to be done to make transitions to adulthood better.

The strategy was developed in conjunction with health, social care and education professionals, young people and their carers.  However, we recognise that this was only a small group and so we are now consulting more widely to ensure that all the required actions have been captured and that we have identified the right priorities.

The public consultation on the Joint Health, Social Care and Education Transitions Strategy is open between 12 November 2018 and 8 February 2019.

We would like to hear the views of professionals who have a role in the transitions process, as well as young people and their carers.

The online consultation survey is available here – please encourage any relevant professionals you work with to complete the survey, as well as young people and their carers.  It is important that this consultation is shared as widely as possible as this will allow us to make sure that the strategy reflects the views of the key stakeholders.
